STEP ONE

 

A.  Club Applications - This is required for your team, give a call if you have questions on which to sign up for, the LEVEL 1 is probably appropriate unless you want to do a "Not for Profit".

 

B.  Multi player application - Use this form to register all kids on one team.  Print off two for both teams.  Be sure to sign the kids up for the $14 membership, this will give them added insurance coverage.  The added coverage comes into play when they play in non-sanctioned AAU events.

 

C.  Non Athlete Application - for your coaches and all bench personnel.  These are individual forms so a separate form is need for each of the personnel.  Have the coaches purchase $16 memberships this will give them additional insurance coverage for the $2 extra.

STEP TWO

 

A.  Gather copies all birth certificates for each player

 

B.  Gather copies all grade/report cards for each player

 

C.  Collect funds ($14 / player & $16 / coach and bench personnel & $30 / club)
